BY SIGNING THIS LEGAL DOCUMENT, YOU WILL WAIVE CERTAIN LEGAL RIGHTS INCLUDING THE RIGHT TO SUE. PLEASE READ CAREFULLY.
I hereby, for myself, my heirs, executors and administrators, waive and release any and all claims for damages I may have against the Badminton Alberta, Millenium Place, City of Sherwood Park, Badminton Canada and their respective agents, members of staff, for any and all injuries which may be suffered by me, through accident or negligence, in conjunction with my competition and attendance at all related activities of the 2009 CANADIAN NATIONAL BADMINTON CHAMPIONSHIPS during the period of time May 3-9, 2009.
I realize that participation in the above mentioned event is conditional upon the receipt of this original signed entry form prior to the entry deadline of the championship.

BY SIGNING THIS LEGAL DOCUMENT, YOU WILL ACCEPT CERTAIN CONDITIONS OF ENTRY TO THE NOTED CHAMPIONSHIP, PLEASE READ CAREFULLY.
I the undersigned, ___________________________________________ , as the parent or legal guardian of the minor _______________________________________, have read and understood the conditions of entry to the 2009 CANADIAN NATIONAL BADMINTON CHAMPIONSHIPS in Sherwood Park and I agree that I carry the full responsibility for that person’s actions while attending the event.
I understand that signing this acknowledgement of the following tournament policy is a condition of entry for the above minor.
• Any illegal purchase, possession or consumption of alcohol or restricted drugs will be reported to the police;
• Destruction of public or private property will be reported to the police;
• Badminton Canada’s Discipline Committee and the Organizing Committee reserve the right to deny participation, without appeal to the minor at any time, if it is deemed reasonable that the words or actions of the minor may lead or have led to injury or are viewed as verbal abuse of another participant or official of the event.

Je soussigné(e), en mon nom, celui de mes héritiers, exécuteurs testamentaires et administrateurs, tiens à couvert et exempt de toute réclamation, relative à des dommages que je pourrais avoir subis, Badminton Alberta, Millenium Place, la Ville de Sherwood Park, Badminton Canada et leurs agents, membres du personnel respectifs, ainsi que toute réclamation relative à toute blessure qui aurait pu m’être infligée, par accident ou négligence, en rapport avec ma compétition ou avec ma participation à toute activité connexe aux Championnats canadien de badminton junior 2009, pendant la période du 3 au 9 mai 2009.
Je reconnais que je ne pourrai participer aux épreuves mentionnées ci dessus que si le comité organisateur reçoit le présent document original dûment signé avant la date limite des inscriptions pour les dits Championnats.

Je soussigné(e), ___________________________________________, parent ou tuteur légal du mineur _______________________________________, certifie avoir lu et compris les conditions d’admission au Championnat canadien de badminton junior 2009 au Millenium Place et accepte d’assumer l’entière responsabilité des actions commises par cette personne pendant le championnat.
Il est entendu que le simple fait de signer la présente reconnaissance de la politique dudit tournoi constitue une condition d’admission du mineur susmentionné.
• Tout achat, toute possession ou toute consumation illégale d’alcool ou de drogues proscrites seront signalés à la police;
• Toute destruction de propriété publique ou privée sera signalée à la police;
• Le comité de discipline et le comité organisateur du Championnat canadien de badminton junior 2009 se réservent le droit de refuser, sans appel et à tout moment, la participation du mineur, si l’on juge raisonnablement que ses paroles ou ses actes peuvent porter ou avoir portés préjudice, ou peuvent être considérés comme un abus verbal envers un autre participant ou un officiel de l’événement.
